WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.95(A) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314656489, CITATION NUMBER 1, ITEM NUMBER 1, ISSUED ON JUNE 3, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT THE CINNABON STORE PROJECT IN THE MICRONESIA MALL, DEDEDO, GUAM.    INTER-TECH BUILDERS, INC. 29 CFR 1926.404(b)(1)(i): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors:    (a) At the seventh floor exit door to the stairs on the south side exterior of the Premium Tower on or about February 20, 2013, there was no assured equipment grounding conductor program or ground-fault circuit-interrupter protection for employees that used a 110 VAC Makita drill to install screws into cement or gypsum boards.  The drill was powered through an extension cord that was plugged into a outlet, located in the hallway on the seventh floor of the Premium Tower.    INTER-TECH BUILDERS, INC. 29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables were not protected from damage:    (a) At the seventh floor exit door to the stairs on the south side exterior of the Premium Tower on or about February 20, 2013, an extension cord was pinched between the bottom of the door and the weather stripping on the floor.  The outside insulation was cut on the cord.  The cord powered a 110 VAC Makita drill, used on the exterior stairway landing, and was plugged into a outlet, located in the hallway on the seventh floor.
