29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   The employer failed to develop an energy control program that includes but is not limited to employee training and documented procedures to control hazardous energy sources when working on equipment such as but not limited to conveyors, baler systems, shears and electrical equipment.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  The employer authorized production employees to operate the powered industrial trucks failing to provide training that included a practical training, (demonstration performed by the trainer and practical exercises performed by the trainee) and an evaluation of the operators performance in the workplace.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).     First America Metal Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(1), which was contained in OSHA inspection number 312724420, citation number 01, item number 003 and was affirmed as a final order on May 7, 2009 with respect to a workplace located at 113 Industrial Dr. Minooka IL 60447
