29 CFR 1926.251(a)(2)(iii): The employer failed to ensure that rigging without affixed, legible identification markings, required by paragraph (a)(2)(i) of this section was not used: (a) Employee(s) operating a forklift to lift stones that weigh between 60 - 600 pounds were using a forklift fork jib attachment (D&D) that measured 4 inches by 6 inches by 6 feet, did not have an identification label or tag that provided the manufacturer's safe working load. The stones were being lifted up to 22 feet high, thus exposing employee(s) to crushing hazards. (b) Employee(s) operating a forklift to lift stones that weigh between 60 - 600 pounds were using a forklift fork jib attachment (D&M) that measured 3 inches by 6 inches by 117 inches, did not have an identification label or tag that provided the manufacturer's safe working load. The stones were being lifted up to 22 feet high, thus exposing employee(s) to crushing hazards.
