The guardrail on the hopper mezzanine near the stair landing was not provided with a midrail or toe board. B. The guardrail on the open side of the stair landing was not provided with a midrail. C. The stairs leading to the hopper mezzanine were not provided with a midrail on the open side.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Block Plant: On or about February 20, 2013, the employer did not provide a safeguard to prevent access to the unguarded wire rope for the skip hoist, used to transfer concrete and aggregate to a block making machine. An employee entered the area and contacted the wire rope, resulting in the amputation of eight fingers. Specific Abatement Documentation is Required for this item.
