On or about February 28, 2018, Block Plant at the Cuber Pallet Injector: On the south side of the equipment, free-standing barrier guards provided to prevent access to moving chains on the pallet conveyor and a pit behind the equipment had been removed. The guards were not affixed to the equipment or to the building's structural members, exposing employees to amputation and tripping hazards.

29 CFR 1910.219(f)(3): Sprockets and chains which were seven feet or less above floors or platforms were not enclosed: A. On or about February 28, 2018, Block Plant, at the Cuber Pallet Injector: 1. On the north side of the, equipment, a 44" length of chain for the pallet conveyor was unguarded, exposing employees to amputation hazards. 2. On the south side of the equipment, free-standing barrier guards provided to prevent access to the chain for the conveyor and a pit behind the equipment had been removed, exposing employees to amputation and tripping hazards.

On or about February 28, 2018, Block Storage Area: An energized orange flexible extension cord was lying across the aisle, subject to damage from forktruck traffic. The had been used to provide power to a truck that had been parked in the next aisle and was not removed from service after use.
