29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repairs, with no obstruction across or in aisles that could create a hazard. On or about February 12, 2013, and times prior thereto, a stairwell landing passageway was not kept in good repairs (raised approximately 1 1/2 inches from adjacent mud house walking-working platform). This condition exposed employees to trip and fall hazards from heights greater than ten feet from the ground.

29 CFR 1910.157(c)(4): The employer did not assure that portable fire extinguishers are maintained in a fully charged and operable condition and kept in their designated places at all times except during use. On or about February 12, 2013, and times prior thereto, two portable fire extinguishers located by the light house and at the access of stairs leading to mud pits respectively, were not maintained fully charged and in operable conditions. This condition exposed employees to fire hazards.
