29 CFR 1910.22(c): Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc. On or about February 24, 2014, the pit surrounding the BOP located under Rig #12 was not covered or protected by guardrails, exposing employees to fall hazards exceeding four feet.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Drill site: On or about February 24, 2014, the opening in the mud house was not secured while an employee was using the chemicals from the mud house. This condition exposed the employees to a fall greater than four feet to the ground below.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: Drill site: On or about February 24, 2014, the strain relief was damaged on high voltage plug in #8 at the plug in panel, exposing employees to fire hazards.
