29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: On or about February 14, 2013, on the north side of the Toolpushers trailer, a 220V nominal power lead on the transformer used to supply power the stellar equipment trailer had its outer insulation jacket sliced opened exposing live conductors. Employees were exposed to electrical shock and/or electrocution.

29 CFR 1910.305(g)(1)(i): Flexible cords and cables were not approved or conditions of use and location: On or about February 14, 2013, the sewage system behind the company mans trailer had two 50-foot orange colored electrical extension cords plugged together. The employer did not ensure that employee(s) followed manufacturers instructions. The employee(s) was exposed to electrical shock/burn.
