29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. At the front of the AK and Mini Parts Warehouse Phase 2 structure on or about February 21, 2013, there was no ground-fault circuit-interrupter protection or an assured equipment grounding conductor program when a black and red, 50 foot extension cord was used to power a 110 VAC Bosch BULLDOG Xtreme Rotary Hammer. Employees used the rotary hammer to chip concrete blocks.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. At the front of the AK and Mini Parts Warehouse Phase 2 structure on or about February 21, 2013, the outer insulation of a black and red, 50 foot extension cord, was not secured to the plug. The cord was used to supply 110 VAC power to a Bosch BULLDOG Xtreme Rotary Hammer. Employees used the rotary hammer to chip concrete blocks.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On the roof level of the AK and Mini Parts Warehouse Phase 2 structure on or about February 21, 2013, there was no fall protection for an employee working near the roof edge. The roof was about 9 feet above the ground.
