29 CFR 1910.212(a)(1):   One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points, rotating parts, flying chips and sparks:    (a)  On or about February 10, 2013, on the Cates folder/gluer machines, the ingoing nip points/ pinch points created by the chain conveyors feed system and the end sprockets for the "nesters" were inadequately guarded in that the existing guards did not extend down the sides enough to prevent employees from contacting the ingoing nip points.
