29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:   a.  Rotary department -  On or about February 22, 2013, the following equipment was inadequately guarded thereby exposing employees to point of operation hazards.     1. Cut to length machine with guard openings of 3/4ths of an inch 2. Rotary die cutter (ROT#1) rear access feed system with guard openings greater than 1 inch 3. Rotary die cutters' (ROT#2&3) interlocked guards could be defeated to allow operation of the machine.  An employee experienced a severe laceration injury on February 21, 2013, while operating a rotary die cutter (ROT #3) with a defeated interlocked guard.
