29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) On or about March 21, 2016, employees in the Rotary Department were required to use the SDM/AZCO, Cut-2-Length, Machine #1 to cut foam. The machine's point of operation was not effectively guarded. Employees were thereby exposed to an amputation hazard. The United Gasket Corporation was previously cited twice for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii). The citations were contained in: i) OSHA inspections number 891577, citation number 1, item number 3 and was affirmed as a final order on July 6, 2013, with respect to the workplace located at 1633 S. 55th Ave. Cicero, Illinois. ii) OSHA inspections number 315157263, citation number 1, item number 4 and was affirmed as a final order on July 22, 2011, with respect to the workplace located at 1633 S. 55th Ave. Cicero, Illinois.
