29 CFR 1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   Shop Floor:  On or about March 1, 2013, the employer failed to develop and implement an energy control program to include machine specific procedures for machines with multiple energy sources, annual reviews by authorized employees, appropriate lockout hardware and training for employees.

Operators would not be able to determine the appropriate load rating for the vehicle.

29 CFR 1910.212(a)(1):   One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Shop, on or about March 1, 2013 the following hazards were observed:  A.  Baldor belt sander did not have ingoing nip points created by the belt guarded, exposing employees to avulsion hazards.  B.  The 6 1/2 inch unused portion of the blade on the Amada band saw was not guarded, exposing employees to amputation hazards.  C. 29 CFR 1910.305(b)(1)(ii):  Unused openings in boxes, cabinets, or fittings were not effectively closed:      Rear wall of the Shop Floor, near the emergency exit:  On or about March 1, 2013, the 230V electrical panel box was missing a breaker/blank, leaving a 2 1/2" wide x 3 1/4" high opening 63 inches from the floor, exposing employees to electric shock hazards.
