29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h): A.) Consolidated Glass Corporation - On or about, 2/22/13, employees were performing lockout/tagout on machinery including, but not limited to, the Schiatti 16 spindle double edger and the Beasna cup machine R-8. The servicing and maintenance included, but was not limited to, repairs and preventative maintenance. The company had lockout/tagout procedures but they were deficient in that they did not include steps for notifying affected employees, verification of deenergization, location of disconnects, release from lockout and means for isolating energy sources.

Consolidated Glass Corporation - On or about 2/22/13, maintenance employees utilized dust mask when performing grinding operations but had not received a copy of Appendix D of the Respirator Standard.
