Consolidated Glass Corporation - On or about, 2/22/13, employees were performing lockout/tagout on machinery including, but not limited to, the Schiatti 16 spindle double edger and the Beasna cup machine R-8.  The servicing and maintenance included, but was not limited to, repairs and preventative maintenance.  The company had lockout/tagout procedures but they were deficient in that they did not include steps for notifying affected employees, verification of deenergization, location of disconnects, release from lockout and means for isolating energy sources.
