Consolidated Glass Corporation, loading area, on or about October 26, 2021: The storage of glass panels on the glass transportation truck created a hazard to employees.  During loading operations, too many glass panels were stacked on the driver's side of the truck, causing the storage rack to become unbalanced. When the employee began to secure the load, the glass panels fell, striking and injuring the employee.
