29 CFR 1910.265(c)(18)(i): Pulley(s), sprocket(s), sheave(s), drum(s) and block(s) of conveyor(s) were not arranged to prevent the possibility of injury due to hands or parts of clothing being caught between the belt(s) and pulley(s) or chain(s) and sprocket(s): a) End-Feed to Scragg Trimmer: On or about February 26, 2013 and at other times prior thereto the employer exposed employees to amputation/caught-in hazards in that employees were allowed to work within 1' of an exposed ingoing nip point. The ingoing nip point was located on a rotating shaft at the end-feed belt to the scragg trimmer. b) Unguarded chain and sprocket under Greenline belt; On or about February 26, 2013 and at other times prior thereto the employer exposed employees to caught-in hazards in that employees were allowed to walk under an exposed rotating chain and sprocket that was located 6'-1" from the ground below. Exposed employees use the area below the exposed chain and sprocket as a walkway. c) Bruwco resaw return line; On or about February 26, 2013 and at other times prior thereto the employer exposed employees to amputation/caught-in hazards in that employees were allowed to work within 1' of an exposed rotating chain and sprocket located on the Bruwco resaw return line. Exposed employee was laying a straight edge atop the cants to ensure the cants would receive a straight-line cut. d) Bark loadout conveyor system; On or about February 26, 2013 and at other times prior thereto the employer exposed employees to amputation/caught-in hazards in that employees were allowed to work within 1' of an exposed rotating chain and sprocket located at the end of the bark loadout system conveyor. Exposed employee was required to shovel wood shavings from the ground onto the end of the conveyor.

29 CFR 1910.1030(f)(1)(i): The employer did not make available the hepatitis B vaccine and vaccination series to all employees who have occupational exposure: a) Sawmill; On or about February 26, 2013 and at times prior thereto, the employer exposed his employees to potentiallly becoming infected with hepatitis B in that employees who had been trained in CPR/1st Aid and were expected to rendered CPR/1st Aid in case of an accident had not be offered the hepatitis B vaccination series as required by the regulation.
