29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      (a)  Facility:  On or about February 20, 2013, the employer did not have written lockout/tagout program.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:      (a)  Jobsite:  On or about February 20, 2013, the company did not have any written lockout/tagout procedures for any of the covered equipement (powered conveyors, sorting machines, the hog,  saws, powered industrial trucks, etc.) at the facility.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) Facility, Hand Feed Station #2, Between Clipping and Grading:  On or about January 17, 2013, an employee got her hand caught in the power rollers while she ws trying to unjam a bundle of veneer hardwood.        (b) Facility, Hand Feed Station #2:  On or about February 20, 2013, the existing guard over the conveyor roller would not prevent an employee from reaching into the area where an accident occurred that resulted in an employee being  slightly injured by the rollers.
