29 CFR 1910.27(c)(4):  The distance from the centerline of rungs, cleats, or steps to the nearest permanent object in back of a ladder was not less than 7 inches:                                                                                              a)  Hog Pit entrance, 119 AID Drive, Darlington, Pennsylvania 16115:  Observed on or about July 09, 2013, the distance between a white drain pipe and the top rung of the fixed ladder leading into the hog pit was less than 7 inches.  The lack of space between the back of the top rung of the ladder and the drain pipe created a tripping hazard for maintenance employees using the ladder to access the hog pit.

29 CFR 1910.27(d)(3): The side rails of through or side-step ladder extensions did not extend 3 and 1/2 feet above parapets and landings:                                                                                             a)  Hog Pit entrance, 119 AID Drive, Darlington, Pennsylvania 16115:  Observed on or about July 09, 2013, the side rails of a fixed ladder did not extend 3 and 1/2 feet beyond the upper landing surface.  Maintenance employees were exposed to a fall hazard of approximately 8 feet to the cement surface below while using the ladder.

29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:                                                                                              a)  Warehouse and production areas, 119 AID Drive, Darlington, Pennsylvania 16115:  Observed on or about June 25, 2013, skids of veneer were not stacked, blocked, interlocked, or limited in height to prevent them from sliding or collapse.  Employees were exposed to struck-by and crushed-by while moving veneer skids by forklift and while using ladders to search for specific skids.

29 CFR 1910.178(q)(1):  Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:       a)  Warehouse and production areas, 119 AID Drive, Darlington, Pennsylvania 16115:  Observed on or about June 25, 2013, the employer did not ensure defective forklifts were removed from service and repaired.  Forklift #13 was kept in service after a deficiency with the parking brake was identified during preventative maintenance and pre-shirt inspections.  Forklift drivers and employees working in the production and warehouse areas were exposed to struck-by and crushed-by hazards.

29 CFR 1910.215(a)(1): Abrasive wheel(s) on grinding machines were not used with safety guard(s):      a)  Maintenance shop, 119 AID Drive, Pittsburgh, Pennsylvania 16115:  Observed on or about July 9, 2013, a 10 inch bench grinder located in the maintenance shop was not equipped with a tongue guard.  Employees using the abrasive wheel on the bench grinder were exposed to struck-by and fire hazards created by the lack of a tongue guard.
