Autoplas Division, Windsor Mold USA Inc. located in Bellevue, Ohio: On or about February 27, 2013, the employer did not ensure the control buttons, for the 50 ton main hoist on the EMH crane #3 located at the east end of the production department, were labeled as to their function.
