29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Autoplas Division, Bellevue, Ohio; On or about September 8, 2015 the employer failed to ensure employees were protected from fall hazards while attaching swivel eye hooks to molds in the mold storage area. Employees were exposed to falls of approximately 15-20 feet when climbing on top of molds stack three high to attach swivel eye hooks for overhead crane anchorage points.
