On or before February 12, 2013, the employer did not ensure that proper guarding was provided to protect machinists and other employees from hazards created by unguarded point-of- operation on each Index milling machines used by employees in the shop. This condition exposed employees to a strike-by and/or amputation hazard.    (c) Magna Energy Services, LLC @ 0902 Road 4, Wiggins, CO. On or before February 12, 2013 the employer did not ensure that the DoAll Horizontal Metal Band Saw used by employees in the shop was equipped with adequate point-of-operation blade guarding. The unused portion of the front the blade was not guarded to protect employees against amputation hazards during the operating cycles.    (d) Magna Energy Services, LLC @ 0902 Road 4, Wiggins, CO. On or before February 12, 2013 the employer did not ensure that the Horizontal Metal Hack Saw used by employees in the shop was equipped with adequate point-of-operation blade guarding.
