29 CFR 1926.20(b)(2):     The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them):     On or about February 25, 2013, an employee was exposed to a fall from height hazard of approximately 20 feet to the concrete ground below, while working from a Genie S-60 Aerial lift, and not having the required lanyard attached.  The supervisor failed to perform frequent and regular inspections of the job site to identify this hazardous operation.
