29 CFR 1926.453(b)(2)(v):     A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:      a)  On March 1, 2103, at the address of 995 Enterprise Ave. Wauseon, OH:  The employer did not assure employees working from aerial lifts were adequately protected from falls.  The foreman and laborers were performing work in two separate aerial lifts without fall protection.  The employees utilizing the aerial lifts were exposed to a 15 foot fall to the pavement below.      Deke Enterprises, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 313786238, citation number 1, item number 2 and was affirmed as a final order on January 24, 2011, with respect to a workplace located near 2720 Crossroads Parkway, Rossford, Ohio.
