29 CFR 1926.251(a)(1):     Defective rigging equipment was not removed from service:    Jobsite:  Front of building    Rope used to hoist planks being used for scaffold erection was not thoroughly inspected before use.  The hoisting rope was defective and failed, causing a plank to fall to the ground.
