The employer had not received the manufacturer's approval for this attachment.

29 CFR 1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: 1) On February 27, 2013, and at prior times, the employer maintained a manlift basket that was used with forklifts to change lightbulbs. The employer had not marked the fork lift with the identity of the attachment and the weight of the truck and attachment combination.

29 CFR 1910.212(a)(2): Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible: 1) On February 27, 2013, a guard for a chain and sprocket on the Greer Line had been removed. The guard was not configured so that it could be securely affixed to the machine.
