29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about December 8, 2016, and at times prior, the employer did not develop, document, and utilize procedures for the control of potentially hazardous energy when employees were engaged in the servicing, maintenance and cleaning of a caramel depositor machine. Employees were exposed to amputation hazards in that the caramel depositor machine, also known as the "bear machine", was serviced, maintained and cleaned without isolating its energy source. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

Employer's energy control procedure violates this OSHA standard in that it requires that employees put a padlock on the access door of electrical panel C as part of their lockout/tagout program.
