29 CFR 1910.146(c)(4):  When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146:     a.  On or about February 25, 2013, at the batch plant, employees were working inside a concrete mixer without a permit confine space program, exposing themselves to caught-by hazards.    b.  On or about February 25, 2013, at the CDS facility, employees were entering the concrete truck mixers without a permit confine space program, exposing themselves to caught-by hazards.

29 CFR 1910.179(d)(4)(iii):  Fixed ladder(s) on crane(s) were not constructed in compliance with 29 CFR 1910.27:     a.  On or about February 25, 2013, employees operating the 60-ton gantry crane were using defective ladders that were permanently affixed to the crane, exposing themselves to a fall hazard.    b.  On or about February 25, 2013, employees operating the 40-ton gantry crane were using defective ladders that were permanently affixed to the crane, exposing themselves to a fall hazard.

29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a.  On or about February 25, 2013, on the east side of the building, the bender portion on the ironworker did not have the point of operation guard, exposing employees to an amputation hazard.    b.  On or about February 25, 2013, on the east side of the building the shear portion on the ironworker did not have the point of operation guard, exposing employees to an amputation hazard.    c.  On or about February 25, 2013, on the east side of the building, the punch portion on the ironworker did not have the point of operation guard, exposing employees to an amputation hazard.    d.  On or about February 25, 2013, in the steel shop, the Slugger magnet drill press was missing the bit guard, exposing employees to flying debris.

29 CFR 1910.213(h)(1):  The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the material:     a.  On or about February 25, 2013, on the west wall in the storage area, the miter saw did not have the lower blade guard, exposing employees to an amputation hazard.
