29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder�s length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a)Residential Construction Site: Employer failed to ensure employees used portable ladder correctly by extending ladder 3 foot above roof edge while using ladder to access roof top to performing roofing job tasks.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards. (a) At the rear of the house, the employer exposed employees to falls of 9 feet 4 inches from ladders where no ladder training was provided to employees prior to using ladders to access the roof. The ladder was not long enough to extend 3 feet beyond the landing surface.
