29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection: On or about March 6, 2013, at the shop, employees required to perform servicing and/or maintenance operations on equipment such as tractors and forklift trucks were exposed to the hazards associated with the release of hazardous energy. The employer did not certify that the periodic inspections had been performed.

The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed.
