29 CFR 1910.146(c)(2):  The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:    On 03/06/2013, the employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence of and the danger posed by:    a)  Three (3) silo access spaces which were horizontal permit spaces located directly below the 1st floor silos #1, #2, and #3, in the Drying Department.    b)  A loaf slurry blender located on the Blender Deck.    NOTE: A sign reading DANGER-PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language would satisfy the requirement for a sign.    ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED.

29 CFR 1910.146(k)(2)(iv):  The employer did not ensure that affected employees practiced making permit space rescues at least once every 12 months, by means of simulated rescue operations in which they removed dummies, manikins, or actual persons from actual permit spaces or representative permit spaces:    a)  On 03/06/2013, the employer did not ensure that each Maintenance employee had practiced making non-entry permit space rescues from vertical and horizontal spaces at least once every 12 months from actual or representative permit spaces.    ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED.
