(a) Blackeagle Energy Services, 230 Commerce Drive, Berthoud, CO 80513; On and before March 4, 2013, the exposing employer did not ensure that a fork lift driver negotiating turns reduced the forklifts operating speed to a safe level.  Hazard resulted in the forklift running over an employees leg as it negotiated a turn on a muddy, snow bound dirt lane due to the drivers failure to reduce the powered industrial vehicle to a safe speed and slowly ease into the turn, which was also on a slight decline.

(a) Blackeagle Energy Services, 230 Commerce Drive, Berthoud, CO 80513; On and before March 4, 2013, the exposing employer allowed an attachment, composed of a D-clamp, block, and slings, to be used to lift pipe which was not provided by the manufacturer but rather assembled at the site and thus not properly centered or secured on the mast.  Condition resulted in the load shifting during transit and the forklift running over an employees foot and leg as he assisted in attempting to secure/stabilize the load.
