29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about March 6, 2013, the aeration fan located outside at ground level of bin # 8 had a damaged fan guard. This condition exposed the employees to potential caught-between, and/or amputation hazards.

29 CFR 1910.272(j)(2)(ii): The employer did not immediately remove any fugitive grain dust accumulations whenever the exceeded 1/8 inch (.32cm) at priority housekeeping areas, pursuant to the housekeeping program for grain elevators: On or about March 6, 2013, in four locations, the grain elevator had accumulation of dust exceeding the allowable 1/8 inch. This condition exposed the employees to explosion hazards. 1. The motor driving the conveyor belt, located on the south side of the tunnel, had approximately 2 1/2 inches of dust accumulation on top. 2. The floor under the conveyor belt , located on the south side of the tunnel, had approximately 2 1/2 inches of dust accumulation. 3. The floor under the north side tail pulley conveyor had an accumulation of Milo/dust. The accumulation pile was approximately 9 inches high. 4. The boot pit floor on the north end had an accumulation of Milo/dust. The accumulation pile was approximately 9 inches high.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: On or about March 6, 2013, the light switch cover plate located at the southside work floor area was loose. This condition exposed the employees to an electrical hazard.
