OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to struck by hazards.   On or about August 13, 2014, the employer did not ensure the 1-ton Hoist located in the maintenance shop was inspected for defects to include deformed hooks, exposing employees to falling loads or struck by hazards.

29 CFR 1910.37(b)(5): Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g. closet).  On or about August 13, 2014, the employer did not ensure that all doors located in the maintenance shop along a passageway or access route that was not an exit, was identified as "Not an Exit" or its actual use exposing employees to possible entrapment and fire hazards.

29 CFR 1910.219(c)(4)(ii): The unused keyway(s) of projecting shaft end(s) were not filled up or covered.  On or about August 13, 2014, the employer did not ensure the projecting shaft ends located in the boot pit were guarded by caps due to the shafts having unused keyways, exposing employees to being caught-in the rotating parts.
