Ameriwood Industries, Inc. located at 458 Second Ave. Tiffin, OH; On or about March 8, 2013, the employer did not assure employees disconnected the power to the Big W Industries Foam Cutter before removing small foam pieces from the table. Employees are required to grab small foam pieces from the cutter exposing them to burn hazards.  Ameriwood Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d)(3) which was contained in OSHA inspection number 311603179, citation number 1, item number 5a and was affirmed as final order on 9/2/2008, and inspection number 311606880, citation number 2, item 4a, and was affirmed as final order on 7/29/2009 with respect to a workplace located at 458 Second Ave. Tiffin, OH.
