29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:    Ameriwood Industries, Inc. - Tiffin, Ohio: On or about February 10, 2016, the employer did not ensure that employees who service the CNC drill and associated equipment such as conveyors and stackers were adequately trained in the skills required for the safe application, usage, and removal of energy controls.  Employees that perform tasks such as set-up, cleaning, and maintenance of the equipment, were not trained to lockout the machine to protect against unexpected start-up.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    a. Ameriwood Industries, Inc - Tiffin, Ohio: On or about February 10, 2016, an employee was exposed to crushing injuries while servicing the Bargstedt in-feed stacker(s) for the CNC drill.  An employee unjamming the in-feed stacker by removing cardboard product from a stack of product on the roller conveyor sustained crushing injuries when a stacker arm, which feeds wood panel product to the machine, lowered onto the employee.  The employer failed to implement energy control application steps to isolate the equipment from the hazardous energy source(s).    b. Ameriwood Industries, Inc. - Tiffin, Ohio: On or about April 5, 2016, employees were exposed to crushing hazards in that the Bargstedt in-feed stackers located at the CNC drill  were not locked out prior to servicing the stacker arms.  Prior to accessing the hazard area to level the stacker arm(s), the employer failed to implement energy control application steps to isolate the equipment from the hazardous energy source(s).    c. Ameriwood Industries, Inc. - Tiffin, Ohio: On or about April 19, 2016, employees were exposed to crushing hazards in that the Bargstedt in-feed stackers located at the CNC drill  were not locked out prior to servicing the stacker arms. Prior to accessing the hazard area to level the stacker arm(s), the employer failed to implement energy control application steps to isolate the equipment from the hazardous energy source(s).    d. Ameriwood Industries, Inc. - Tiffin, Ohio: On or about April 22, 2016, employees were exposed to crushing and amputation injuries in that the Weeke 1 (s/n 0-251-19-0011) CNC drill was not locked out prior to set-up activities. Prior to accessing the hazard area to install drilling bits, the employer failed to implement energy control application steps to isolate the equipment from the hazardous energy source(s).    e. Ameriwood Industries, Inc. - Tiffin, Ohio: On or about April 25, 2016, employees were exposed to crushing and amputation injuries in that the Weeke 2 (s/n 0-259-08-1814) CNC drill was not locked out prior to set-up activities. Prior to accessing the hazard area to install drilling bits, the employer failed to implement energy control application steps to isolate the equipment from the hazardous energy source(s).    f. Ameriwood Industries, Inc. - Tiffin, Ohio: On or about April 25, 2016, employees were exposed to crushing and amputation injuries in that the CNC drills and associated conveyor equipment were not locked out prior to maintenance activities.  Prior to accessing the hazard area for cleaning operations, the employer failed to implement energy control application steps to isolate the equipment from the hazardous energy source(s).
