29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Facility - the employer did not develop and implement a program for the control of hazardous energy (lockout/tagout) where employees performed maintenance of electrical equipment including replacement of switches. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.303(f): Each service, feeder and branch circuit, at its disconnecting means or over current device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:(a) (LOCATION) (IDENTIFY EQUIPMENT) (STATE HAZARD(S) TO EMPLOYEE(S)) a) Kitchen - Workers operated two adjacent switch panels to power the lights, fan, dishwasher, and wall outlets. The switches were not clearly labeled and their functions could be identified. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
