29 CFR 1910.243(c)(1): Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4)(a): On or about March 8, 2013, An angle grinder used for removing fiberglass from countertops was missing its guard, exposing employees the hazards of being caught in the spinning wheel or struck by debris of an exploding wheel. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that grinders are used with their guarding is in place.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure On or about March 8, 2013, flexible cords were used as permanent wiring, exposing employees to burn and shock hazards. a) An extension cord reel above the engraving table was powered by an extension cord that was affixed to the structure. b) An extension cord along the south wall was used to power a router and was permanently affixed to the conduit along the wall. c) An extension cord ran up the north wall, across the ceiling, and down the south wall to power a wall mounted fan and a swamp cooler. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that flexible cords are not affixed to building surfaces and used as permanent wiring.
