29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  On or about 13 April 2017, two employees were walking on a wet floor that contained a protruding pvc water pipe protected by wooden boards that ran across a frequently accessed pathway, exposing the employees to a trip and fall hazard.

29 CFR 1910.25(b)(7): Standard stairs were not used to provide access from one walking-working surface to another where operations necessitated regular and routine travel between levels.  On or about 13 April 2017, and at times prior thereto, two employees were using blocks, in-lieu of standard stairs, to routinely access the dock area at the back of the facility, exposing the employees to trip and fall hazards.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  On or about 13 April 2017, and at times prior thereto, two employees were operating a Hyster forklift Model: H50CT without successfully completing the required training and evaluation, exposing the employees to hazards such as being struck by or crushed by the powered industrial truck in the event of a tip over.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met.  1910.1200-(f) Labels and other forms of warning;                  (g) Safety data sheets;                  (h) Employee information and training;  On or about 13 April 2017, the employer did not develop or implement an effective written hazard communication program at the workplace where employees were using hazardous chemicals such as but not limited to Silica, Stone Shield Polyester Adhesive, Benzomatic Mapp Gas, and Stone Weld Epoxy Adhesive, exposing the employees to the hazards such as, but not limited to inhalation, absorption, and dermal contact.

29 CFR 1910.134(c)(2)(i): When the employer determined that any voluntary respirator use was permissible, the employer did not provide the respirator users with the information contained in Appendix D to this section ("Information for Employees Using Respirators When Not Required Under the Standard");  On or about 13 April 2017, and at times prior thereto, employees were voluntarily using N-95 respirators when performing cutting and grinding operations on the marble slab without the employer providing employees the information contained in Appendix D of the respiratory standard, exposing the employees to respirable silica inhalation hazards from improper use or storage of the respirators provided.
