29 CFR 1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire  (a) On or about March 8, 2013, at 2661 Channel Ave, Memphis, TN - Oxygen compressed gas cylinder was improperly stored  with flammable materials on the barge deck exposing employees to a fire hazard.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1915.55(a)(1): Valve protection caps on gas Welding and cutting cylinders were not in place or secure.   On or about March 8, 2013, on a barge located at 2661 Channel Ave, Memphis, TN - employees were exposed to struck by incidents from compressed gas cylinders (acetylene and oxygen) that were without valve caps in place .   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1915.55(a)(9): Acetylene cylinders were not secured in an upright position.  a) On or about March 8, 2013, on a barge, located at 2661 Channel Ave, Memphis, TN - employees were exposed to fire hazards from compressed gas cylinders (acetylene and oxygen) that were not secured in an upright position. The compressed gas cylinders were observed laying on their side on the barge with evidence of foot prints (in the sand) all around in the cylinders.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

The fire extinguisher was not properly mounted for employee accessibility and there were no monthly inspections conducted.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
