29 CFR 1926.453(b)(2)(vi):  Boom and basket load limits specified by the manufacturer were exceeded:    a)  Job Site:  59 Waterville Road, Avon, CT. - On or about 3-11-13 - Three (3) employees were observed being lifted and working from a Genie 4WD Boom Lift.  Rated capacity for the lift is 500 lbs.

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a) Job Site:  59 Waterville Road, Avon, CT. - On or about 3-11-13 - Employee was observed working in the tower structure 25 feet above the ground with no fall protection system ibeing used.

29 CFR 1926.502(d)(16)(iii):   Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    a)  Job Site: 59 Waterville Road, Avon, CT. - On or about 3-11-13 - Employees were observed working in a 4WD Genie Boom lift not wearing proper personal fall protection systems which exposed them to fall hazards of 15 to 20 feet to wooden beam and flooring surfaces below.  Fall system in use consisted of a 6 foot shock absorbing lanyard attached with a rope grab device to a 6 foot lenght of lifeline wrapped around the railing system of the lift.

29 CFR 1926.503(a)(2)(ii):  The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used:    a)  Job Site:  Watervill Road, Avon, CT. - On or about 3-11-13 -  Employees were observed working in an aerial lift using fall protection systems that were designed to be used on a roof installation.  6 foot lanyard with shock absorbing system with rope grab lanyard and lifeline wrapped around the railing of the aerial lift.
