29 CFR 1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    This violation occurred on March 11, 2013, when the employer did not establish a program consisting of energy control procedures which includes but is not limited to employee training and periodic inspections.

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields.    This violation occurred on March 11, 2013, when the employer did not ensure that employees in the production area where welding activities are performed were protected from the welding rays.
