29 CFR 1910.134(c)(1): The employer did not establish and implement a written respiratory protection program with worksite-specific procedures in any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer:   This violation occurred on May 13, 2013 at an establishment located at 159 Putman St. Sulphur Springs, TX 75483; where one (1) employee was required to wear a negative pressure half face piece and full face piece respirator for protection against sandblasting operations containing crystalline silicon dioxide compounds and against other nuisance dusts.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met.   This violation occurred on May 13, 2013 at an establishment located at 159 Putman St. Sulphur Springs, TX 75483; where at least one (1) employee was exposed to skin contact and inhalation hazards during spray painting operations and tasks involving the use of spent abrasive blast media such as xylene, silicon dioxide, titanium dioxide, and other nuisance dusts. No written program had been implemented to communicate chemical hazards and protection measures to employees.
