(a)Casson Duncan Construction @ 13598 Via Varra St., Broomfield, CO 80020: On and before March 8th, 2013, the controlling employer did not ensure that subcontractor employees were utilizing portable ladders in such a way that enabled them to access and egress from work areas using ladders that extended 3' above the walking working surface.  Employees were using a portable extension-ladder for access to the roof. Employees were exposed to a fall of approximately 10' while attempting to use the ladder.
