29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:    (a) Stairway, new townhouse construction located at 3367 Vintage Circle, Smyrna, GA 30080: Employees were exposed to falls of approximately 10'4" to the concrete surface below when a temporary stair rail was not provided to employees using the 16 riser high staircase to access the second floor of the new townhome.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    (a) Front side, new townhouse construction located at 3367 Vintage Circle, Smyrna, GA 30080: Employees were exposed to eye injuries while operating pneumatic PNEU Tools Tico Nailer and Makita (Model 5007F) 7 1/4" Circular Saws engaged in framing activities with no safety glasses.        Silent P Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 741181, citation number 1 , item number 1 and was affirmed as a final order on 01/10/2013, with respect to a workplace located at 1019 Belmont Commons Drive, Smyrna, GA 30080.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    (a) Front top plate, new townhouse construction located at 3367 Vintage Circle, Smyrna, GA 30080: Employees were exposed to fall hazards of approximately 19'1" to the ground while installing roof trusses from the top outside plates with no fall protection system in place.      Silent P Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 741181, citation number 1, item number 2 and was affirmed as a final order on 01/10/2013, with respect to a workplace located at 1019 Belmont Commons Drive, Smyrna, GA 30080.
