29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) At the site located at Converse and Drew Streets, Houston, TX: On March 4, 2013 an employee beneath a roof edge 34 feet above where roofing work was being done with hammers, nails and roofing materials was not protected by a helmet, exposing the employee to a head injury.
