On or about June 3, 2016, on the southeast side of a 5/12 pitch roof of a new residential building, an employee installing asphalt shingles to the roof was wearing a personal fall protection systems with a lifeline that was to long to prevent the employee from hitting the lower level, exposing the employee to a 10 feet 3 inch fall hazard.
