29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a.  On or about June 3, 2016, on the southeast side of the residential building, a superintendent installing asphalt shingles to a roof while using a pneumatic nail gun did not wear eye protection, exposing the employee to the hazard of being struck in the eye or face by flying debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a.  On or about December 15, 2015, on the north side of the residential house, two employees working on a 5/12 pitch roof was not protected from a 14-feet 1-inch fall hazard by the use of a personal fall protection system.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a.  On or about June 3, 2016, on the southeast side of a 5/12 pitch roof of a new residential building, an employee installing asphalt shingles to the roof was wearing a personal fall protection systems with a lifeline that was to long to prevent the employee from hitting the lower level, exposing the employee to a 10 feet 3 inch fall hazard.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a.   On or about June 3, 2016 on the southeast side of a new residential building, the ladder used to access the roof did not extend 3 feet above the upper landing surface, exposing employees to a 10 feet fall hazard.
