Combined Systems Inc., Penn Arms Division - On or about 3/12/13, employees were exposed to crush hazards from being ejected from powered industrial trucks that were not equipped with proper safety equipment. Forklift (Hyster XLZ; Serial #B187V04679M) was not equipped with an operator restraint system (i.e. seat belt). Employees operated the forklift for activities that included but were not limited to loading/unloading trucks

The start switch consisted of a lever that could be accidentally bumped/snagged. B.) These fans were not designed for industrial environments. C.) Combined Systems, Inc., Penn Arms Division; paint room - On or about 3/12/13, general "household type" heaters were being utilized in the working environment. These heaters were not designed for industrial environments.
