Combined Systems Inc., Penn Arms Division - On or about 3/12/13, employees were exposed to crush hazards from being ejected from powered industrial trucks that were not equipped with proper safety equipment.  Forklift (Hyster XLZ; Serial #B187V04679M) was not equipped with an operator restraint system (i.e. seat belt).  Employees operated the forklift for activities that included but were not limited to loading/unloading trucks

Combined Systems, Inc., Penn Arms Division; paint room - On or about 3/12/13, the lathes start/stop switch was not designed and installed to prevent unintentional activation of the machine.  The start switch consisted of a lever that could be accidentally bumped/snagged.      B.)  Combined Systems, Inc., Penn Arms Division; paint room, production area, annealing area - On or about 3/12/13, general "household type" fans were being utilized in the working environment.  These fans were not designed for industrial environments.   C.) Combined Systems, Inc., Penn Arms Division; paint room - On or about 3/12/13, general  "household type" heaters were being utilized in the working environment.  These heaters were not designed for industrial environments.

Compressed air shall not be used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment.      A.)  Combined Systems, Inc., Penn Arms Division;  - On or about 3/12/13, compressed air for blowing off parts and work areas was set at 70 p.s.i.  These air guns were located in areas including, but not limited to, the production area and the paint room.
