29 CFR 1926.102(a)(1): Eye and face protective equipment were not provided when machines or operations presented potential eye or face injury: (a) Zeigler Construction LLC at 1111 Mountain Home Dr, Fort Collins, CO: On March 11, 2013 the exposing employer did not ensure that two employees utilizing pneumatic nail guns were provided eye protection during the installation of shingles. This condition exposed employees to the hazard of a projectile to the eye.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: (a) Zeigler Construction LLC at 1111 Mountain Home Dr, Fort Collins, CO: On March 11, 2013 the exposing employer did not ensure that each employee engaged in roofing operations were trained to recognize the hazards of working at elevation and trained in the use of fall protection. Employees engaged in roofing activities on a two story residential home, with a roof pitch of 6-in-12 on the top level, were incorrectly utilizing fall protection, in that two employees connected their respective lanyards to a side Dee-ring located at the waist on their fall harness. This condition exposed employees to injury due to the failure of attaching the lanyard to the required attachment point on the fall harness.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed: (a) Zeigler Construction LLC at 1111 Mountain Home Dr, Fort Collins, CO: On March 11, 2013 the exposing employer allowed employees to use a stepladder to access the second level roof from the first level roof. The stepladder was used contrary to manufacturer's instructions, in that the front and back sections of the stepladder was in a closed position when used. The first level roof pitch was 4-in-12. This potentially exposed employees to a fall hazard of approximately 10 feet from the first level roof to the ground.
