29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in 29 CFR 1910.23(e)(3)) on all open sides except where there was entrance to a ramp, stairway, or fixed ladder:  This violation was observed on February 20, 2013, above the maintenance department; where at least one (1) employee, who was walking / working on a catwalk, was not protected from falling approximately 30 1/2-feet to the lower level.
