29 CFR 1926.200 (g)(2):  All traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual on Uniform Traffic Control Devices (MUTCD), 1988 Edition, Revision 3 or Millennium Edition:  a) On or about March 13, 2013, and at times prior, employees were exposed to struck-by hazards associated with moving traffic in that an effective traffic control zone had not be set up around the work area to protect workers due to the following:       1)  Signs and channelizing equipment were not sufficiently instructive to safely move traffic around the work zone in the middle of South Troy Street.   2) Traffic control devices were not set up in accordance with the traffic control plans.   3) The traffic control plan developed by the employer and approved by the city was not available onsite.         Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").  Among other methods, feasible and acceptable methods to correct this hazard would be to provide flaggers, or to set up traffic control zones in accordance with Part 6 of the Manual on Uniform Traffic Control Devices (MUTCD-2003) or more current edition.  The employer could also follow the traffic control plans that they had developed for this type of work.
